Below you will find the Credit Union's online applications. We hope that you will be able to use these applications from the comfort of your home instead of having to pick one up at one of our branches. In order to process most of these forms we require a valid signature therefore the process is not instant. Instead, we ask that you complete the form, print it out, sign it and mail or fax it back to us. The only applications we do not require a signature on are our loan and e-newsletter applications. You will sign loan papers at closing.
We have provided two versions of each application. Both documents contain the same information. One is a PDF, and the other is a DOC. You must have Adobe Acrobat Reader 4.0 or higher in order to use the PDF. You can download this software for free by going to Adobe Online. In order to open the DOC file you must have Microsoft Word installed on your computer. Just click on the appropriate version.

Electronic Services
Use this application to enroll for our online banking product (FedNet 24) or our audio response system (Allegiance Anytime). Both services are completely free; all you need to do is enroll. We require a valid signature in order to set you up. You may mail, fax, or bring in a signed and completed application.

Loan Application
Our online loan application allows you to apply for a loan in real time. Your application results are sent encrypted to our loan officers. We will require a valid signature from you at a later date if you are approved for a loan.

Transfund Check Card Application
Please use this application to request a Transfund Check Card. We do require a valid signature to order you a card. You may mail, fax, or bring in a signed and completed application. It generally takes 7-10 business days from the time we order your card for you to receive it by U.S. mail.

Address Change Application
Please use this application to change the address or email address listed on your account. We require a valid signature in order to process this application. You may mail, fax, or bring in a completed and signed application. Our fax number is (405) 491-6121.

Password/PIN Change Request
Please user this application to request a password/PIN change if you have forgotten your password to access FedNet24, or FedLine24. We require a valid signature in order to change your password/PIN number. You may mail, fax, or bring in a completed and signed application.
